Month to Month - Membership Cancellation/Refund Request

To request a cancellation and refund of the balance of your pre-paid OPERS Facility Usage Membership, you must submit a "Membership Cancellation and Refund Request Form". A $30 administrative fee will be charged for all approved cancellations. Do not use this form to request cancellation of a membership paid through UCSC payroll deduction.

Payroll Deduction Membership Cancellation Request

To cancel a payroll deduction membership, please download the "OPERS Facility Usage Membership Payroll Deduction Cancellation Request Form" and complete, sign and send to the OPERS Business Office. This must be done on the paper form. You cannot cancel online, via e-mail or in-person.
