Month to Month - Membership Cancellation/Refund Request

To request a cancellation and refund of the balance of your pre-paid Athletics & Recreation Facility Usage Membership, you must submit a "Membership Cancellation and Refund Request Form". A $30 administrative fee will be charged for all approved cancellations. Do not use this form to request cancellation of a membership paid through UCSC payroll deduction.

Payroll Deduction Membership Cancellation Request

To cancel a payroll deduction membership, please download the "Facility Usage Membership Payroll Deduction Cancellation Request Form" and complete, sign and send it to the Athletics & Recreation Business Office. This must be done on the paper form. You cannot cancel online, via e-mail or in-person.
